By signing this consent form, I authorize my Employer’s (or prospective Employer’s) Automobile Insurance carrier and Taylor Agency, acting as their broker, to obtain personal information contained in my records with the State Department of Motor Vehicles. I understand that my personal information may be used for the purpose of evaluating my employment. This information may be shared with Taylor Agency, and my employer’s insurance company. I am also authorizing that my personal information may be obtained from time to time by those parties indicated above, as necessary, to evaluate by continued insurability and employability. I understand that my use of a company vehicle in the capacity of employment with my Employer (or prospective Employer) may be restricted or prohibited based on this information. South Carolina motor vehicle reports may be obtained from the website listed below.
